What do I mean?
The whole church in the first centuries of its existence, say from the 1st century to the 4th, was known as catholic. In 325AD the first possible Council following persection occurred. It was the Council of Nicea (in the region of what is now Turkey) and all the leaders of churches gathered from the known world. This was possible after the ceasing of official persecution from the still existing Roman Empire. At this council the leaders of churches in Rome, still the capital of this empire, are seen as desiring some sort of special recognition, but are firmly acknowledged by all as only involved in a limited geographical jurisdiction, so that - for example - the North African churches were not under the Christian leaders of Rome's oversight.
By the time of the Reformation of the 16th century the Roman bishop had assumed oversight over much of Christendom which had not been given by consent or agreement and, also deviated in practise from the early church in significant ways. One such way is by the practise of offering pardon for payment (indulgences). One of the matters the Reformation sought to address.
